What is case management?
The case management program helps individuals with a mental illness obtain and coordinate other resources and can assist with:
- Assessing their needs and strengths
- Paperwork
- Advocating for appropriate services
- Resource navigation and connection to appropriate resources
- Routine follow-up with individuals
- Providing general support for individuals on their road to recovery
Who is eligible for case management?
Case management services are available to all Cumberland-Perry County residents with a mental health diagnosis.
