The Cumberland County Prothonotary's Office is an authorized passport acceptance facility. You may bring a completed form and required documentation to the Prothonotary's Office anytime Monday - Wednesday from 8 a.m. to 4 p.m. and Thursday from 8 a.m. to 1:30 p.m. Appointments are required.

What you will need:

1. Form -
   1. The Prothonotary's Office has blank forms available to the public;
   2. Additionally, forms are available online: Online Passport Application DS-11.
2. Proof of U.S. Citizenship -
   1. State Certified Birth Certificate - Long Form Only;
      • Both parents' names must be included on the birth certificate;
      • To obtain a birth certificate go to PA Vital Records/Birth Certificates or Out of State Vital Records/Birth Certificates;
   2. Naturalization Papers - Original only;
   3. Expired U.S. Passport.
3. Proof of Identity -
   1. Valid Driver's License or State-issued Identification Card - Original.
      • If an applicant, or the parents/legal guardians of a minor, presents an out-of-state driver's license, additional identification is required. Acceptable forms of additional identification include:
        1. School ID;
        2. Health insurance card;
        3. Work ID;
        4. Social security card;
        5. Military ID.
4. Photographs -
   1. 1 color passport photo (2"x2") taken within the last 6 months.
      • Passport photo can be taken in the Prothonotary's Office for a $10 fee.
5. Additional Requirements for Minor Children -
   1. Both parents and the minor child must be present at the time of application.
6. Fees (for each application) -
   1. Passport Books: Check or Money Order made payable to the U.S. Department of State;
      1. $130 if applicant is 16 years of age or older;
      2. $100 if applicant is 15 years of age or younger;
   2. Passport Card: Check or Money Order made payable to the US Department of State in the amount of $30;
   3. Expedited Processing: An additional $60 added onto the payment to the US Department of State;
   4. For each application: Check, Money Order or cash made payable to the Cumberland County Prothonotary in the amount of $35;
   5. For pictures taken by the Prothonotary's Office: Check, Money Order or cash made payable to the Cumberland County Prothonotary in the amount of $10.

Please Note:
All citizenship documentation must be submitted to the US Department of State with the passport application. The citizenship documentation will be returned to you via US mail.
